Learning From Accidents

Welcome to "Learning From Accidents".

Purpose of this website is to provide information about investigation and analysis of accidents and incidents and how to set up a system in their organization to learn from accidents / incidents (and other undesired events).

If you want to learn from events (accidents/incidents), you need at least:

  • have information about event(s)
  • know the causes that led to the event(s)

Of course, you also need to know what to do so learning form events can take place, on an individual level and - more important - on an organizational level. Learning directed at preventing future similar events requires finding the root causes and take measures to eliminate those.

Essential to learning from events is to have a software program that would allow you to:

  • obtain, register and store relevant event information
  • analyse events concerning their causes to also include root causes
  • retrieve information from the database to identify common causes and establish trends

RMS-Incident is a web-based accident and incident investigation software program that can help you reaching the objective: improving from what went wrong by preventing similar events from happening again. Reducing undesired events is one important way to reduce human suffering while, at the same time, also to reduce cost of operation and allow for optimum profit results on an ongoing basis.

RMS-Incident is a web-based software program that is based on the Frank Bird causation or "domino" model.

Methods and software to investigate and analyse accidents are at the heart of the "Learning From Accidents" process. The software that I will discuss is developed by SafetyPort BV in The Netherlands.
