The quality of the accident investigation process can be improved by actually carrying out investigations of
accidents and incidents that have been reported. The quality of the investigations should be measured and feedback,
possibly leading to re-training, provided. A form is provided that will give a simple sample of quality rating of investigation.
Motivation and leadership
Motivation and management leadership is an important driving force behind the learning from accidents process.
This should come from the top of the organization through the various management levels to the operational level.
All levels should be involved depending on the risk classification of the accident or incident which should be
described in the accident protocol. The
leadership should be demonstrated at relevant levels of management to include:
- establishing an effective system of accident / incident reporting, investigation and cause analysis
- allowing budgets for proper training / instruction of management, supervision, specialists and
operational employees
- presence at the location of the accident
- positive reaction to the reporting of incidents
- chairing and/or participating in accident investigation teams
- making available necessary budgets for remedial actions
- follow-up of remedial actions for timely completion
Improving the Accident Investigation Process
Once you have the main ingredients: "Plan - Train - Do", it is a matter of repeating the process to make it
better.
